  • Location: Fruitland, ID (Nampa and Boise locations)

  • Schedule: Full Time


Requirements:



  • Board Certified: Required

  • ID License or IMLC: Required

  • DEA/CSR: Required

  • Comfortable with Regional Anesthesia: Required

  • Must be Comfortable Performing Own Cases or Working in Supervising / Care Team Model: Required

  • Proof of COVID-19 Vaccine: Required (Verbal confirmation at time of name clear)


Practice Details:



  • Seeking a full-time locum Anesthesiologist starting May 2024

  • Work at both Nampa and Boise locations

  • Comfortable with Regional Anesthesia

  • No Cardiac/Complex Peds/Transplants

  • OB Coverage: Required for Nampa, Not Required for Boise


Credentialing Requirements:


Ensure the following items are readily available for the provider at the time of acceptance to expedite the credentialing process:



  • MD Diploma

  • Internship/Residency/Fellowship Certificates

  • All Medical Licenses

  • DEA

  • Board Certification

  • Life Support Cards (BLS)

  • Current and Previous Certificates of Insurance

  • Driver's License

  • Professional Color Photo/Headshot

  • CMEs for the Past 2 Years

  • Immunization Records (TB, Flu, HepB, Tdap, Varicella, COVID)

  • References
