Join a multi-site community health center based in North-side of Chicago. Center has 8 clinic locations in Chicagoland, IL. FQHC with wrap-around services including dental, behavioral health, primary care, gynecology, with on-site pharmacy and labs. Support a diverse patient population.
Medical Site Director delivers quality primary health care services to special populations, emphasizing the prevention and treatment of disease. Provide supervision to Physicians, Mid-Levels, and medical staff. Currently 5 P/.NPs and
2 Physicians. Meets regularly with providers to review care standards and performance, approximately two one-on-one sessions per direct report per month. Measures and monitors the quality of patient care services in Primary Care Services at the assigned clinical location.
Provides direct care for patients supporting patient with preventative and chronic care management. Clinical hours: 28 clinical contact hours and 12 administrative hours p/week. Outpatient clinic hours: Monday - Saturday. Call is 3-4 week annually; phone consults only. Epic EMR.
Requirements:
BE/BC Family Medicine OR Internal Medicine
3 years of clinical experience required
Bilingual (Spanish) language proficiency preferred, not required
IL license, or eligible
Compensation:
Base salary to $280k (first years guaranteed); with option after 9 months to transition to RVU model
Bonus incentives (bi-annually)
