    Northeast Florida Health Services, Inc., a Federally Qualified Health Center (FQHC), is seeking a full-time Family Medicine Physician (MD/DO) to provide high-quality, patient-centered care in our community health centers.
    Responsibilities
    - Provide comprehensive primary care, acute care, and chronic disease management.
    - Deliver preventive care and health screenings tailored to individual patient needs.
    - Collaborate within a multidisciplinary care team to address overall medical and social needs.
    - Maintain accurate, timely documentation in the electronic health record (EHR).
    - Participate in quality improvement and population health initiatives.
    - Ensure compliance with HRSA, HIPAA, and state and federal regulations.
    Qualifications :
    - FL License
    - Experience in FQHC or Community Health
    - Spanish Bilingual a Plus
